Reimagining Family Health Teams with U2Can Care

We are delighted to have this unique opportunity to share how U2CanCare can help existing Family Health Teams expand their services and roster unattached housebound patients and patients facing barriers to accessing a Family Health Team.

The Ontario Government is prioritizing targeted expansion of FamilyHealth Teams. By applying with U2Can Care as an Interdisciplinary Health Provider (IHP), your team can align with the provincial priority to connect two million more people to a publicly funded primary care within four years. Our nurses are prepared to listen to their story and teach them how to simplify day to daycare while keeping all members of the health care team connected. 

Participating Family Health Teams can expect U2CanCare to:

Provide 24/7 non-emergency telephone triage and system navigation from U2Can Care nurse managers

• listen to health concerns (symptom assessment) and direct to appropriate level of care 
• direct scheduling into Family Health Organization/ Team for cases requiring assessment in 24 to 48 hours 
• Referrals to Ontario Health at Home 
• Referrals to pharmacy and allied health care providers
• Dissemination of case specific health teaching resources and public health guidelines 

Act as a direct intake line to facilitate matching unrostered patients to the appropriate Family Health Organization.

• Directing to partnered FHOs and FHTs 

As needed in home intake consultations with a U2Can Care nurse for each newly rostered homebound patient or patients with barriers to accessing the clinic

• Intake forms and home safety assessments submitted to Family Health Team within 24 hours of the home visit 
• As needed follow up in home consultations from a U2Can Care nurse for homebound patients as per physician
